What to expect as a member

As a Healthy Blue Care Together member, you have all the standard Medicaid benefits available to you, plus additional behavioral health services, intellectual and developmental disabilities (I/DD) services, and long-term services and supports (LTSS) to help you be your healthiest.

Care Management

We want to help you in your journey to better health, that’s why as a Healthy Blue Care Together member, you can get care management services at no cost. Your care manager’s goal is to create a plan that will help manage your symptoms and improve your overall health. Your care manager can also:

Offer and explain health information
Create a plan that will help you manage your illness
Help you deal with problems that may prevent you from getting care
Help you prepare for appointments by knowing what to expect
Help you understand when to see your doctor instead of using the emergency room

Healthy Blue Care Together benefits and services

Healthy Blue Care Together will provide or arrange for most services you need. Your health benefits can help you stay as healthy as possible if you:

Are pregnant
Are sick or injured
Experience a substance use disorder or have behavioral health needs
Need assistance with eating, bathing, dressing or other activities of daily living
Need help getting to the doctor’s office
Need medicines

Depending on whether you are a child or young adult, you may have access to certain additional benefits.* If you have questions about in lieu of benefits and extra services, please call Healthy Blue Care Together at 833-777-3611 (TTY 711) 7 a.m. to 6 p.m. Eastern time (including on state holidays).

Note: You must receive the services listed on this page from providers who are in Healthy Blue Care Together’s network. Services must be medically necessary and provided, coordinated or referred by your or your child’s primary care provider (PCP).

Some benefits may have eligibility requirements.

Asthma relief toolkit – Members can choose up to $200 worth of asthma-relief products from a catalog of options
Digital mental health toolkit – Members 13 and older get unlimited access to our web-based online program built on the principles and tools of cognitive behavioral therapy (CBT)
Healthy Adults Healthy Results – Access to online fitness and exercise resources
- A membership to WW® (formerly Weight Watchers)
- $75 toward a gym membership or an at-home exercise kit
Healthy grocery card – $150 healthy grocery card that can be used at a local grocery store or online to purchase healthy foods and beverages
Healthy Kids Healthy Choices – Online virtual nutrition and physical activity support
- $100 for youth club membership to programs such as Boys & Girls Clubs of America, Girl Scouts, Boy Scouts and 4-H
- A $100 stipend to cover the cost of swim lessons or summer camp
Medicine safety kit – Kit includes lockable medicine box, Rx Destroyer gel, childproof prescription caps and pill case covers that reset when opened
Over-the-counter (OTC) and personal care items – Eligible members will receive $50 each quarter to purchase personal care and hygiene products from an online store
Traditional healing benefit – Tribal members will have access to Talking Circles – a culturally-relevant intervention that improves American Indian emotional, spiritual, mental and physical well-being

Baby essentials – Up to $200 worth of items such as a convertible car seat, a portable crib, diapers, a baby monitor, a high chair, a booster seat and baby-proofing items
Childcare assistance – Up to a $225 allowance to use toward childcare services with licensed childcare providers
Educational milestone program – Middle and high school students who send in report cards with a minimum 2.0 GPA are eligible to receive a $25 gift card
GED/TASC/HiSET – Healthy Blue Care Together will cover the cost of a GED, HiSET or TASC high school equivalency test
Post-secondary educational support – Members in 12th grade may receive:
- $100 for textbooks
- $100 toward dorm room items
STEM puzzle set – Eligible members will receive a STEM puzzle set with four different puzzles: science, technology, engineering and math
School supply assistance – $50 toward the purchase of school supplies each year
Sensory solutions – $100 allowance to purchase sensory-regulation tools, such as noise-muffling headphones, gravity blankets, weighted lap pads and stuffed animals, fidget spinners and thinking putty
Summer reading benefit – Eligible members may receive either a $100 gift card to buy books online or in store, or a Kindle and digital library card during the summer months of June, July and August
Toddler essentials – Members can choose two items up to $200, such as bed rails, booster high chair seats, a potty-training kit and a meal package with toddler-sized sippy cups, plates, utensils and placemat
Tutorial services – Members can receive up to 24 hours of tutoring support

Caregiver support – Eligible caregivers may receive a yearly subscription to a sleep and meditation app
Doulas – Pregnant members can receive prenatal, labor and delivery, and postpartum support from a certified doula
Family night package – Eligible members can choose one of two packages to help promote family togetherness
Therapeutic caregiver support – We will offer therapy for foster care, kinship and adoptive caregivers, up to 12 sessions annually

Community transportation – Up to 24 one-way trips annually
Criminal expungement – Healthy Blue Care Together will provide up to $500 to help cover eligible members' court fees, legal fees and criminal record expungement eligibility application fees for criminal record sealing or expungement
Employment basics support package – Members can receive $100 to help support their employment goals
Identification support – Healthy Blue Care Together will cover the cost of obtaining a driver's license, birth certificate, Social Security card or state identification card for members who do not have access to these resources through another source
Transportation essentials – Eligible members can select one of the following transportation options: ride-share gift card, gas card, oil change or public transportation

Emergency preparedness kit and disaster relief funds – Kit includes a multifunction tool, light stick, flashlight, emergency whistle with carabiner, mini first aid kit, emergency survival blanket, waterproof poncho, hand warmers, matches, tissues, towelettes, hand sanitizer and dust mask
Home safety benefit – Up to $100 worth of home safety items such as a gun lock, fire extinguishers and door alarms
Laundromat card – Members can receive $100 annually to help cover the cost of doing laundry
Life essentials kit – Healthy Blue Care Together will offer a monthly stipend to help with food, childcare or transportation needs

New placement welcome wardrobe – Healthy Blue Care Together will provide members up to a $150 allowance for the purchase of essential clothing items, such as socks, shoes, underwear, pajamas and winter jackets
Overnight luggage – Healthy Blue Care Together will provide a 24-inch suitcase to help transitional-age youth carry their personal items as they transition out of the child welfare system
Styling support – Eligible members will receive a $50 styling pass to a hair salon or barber shop and a $50 voucher toward styling education and supplies

Behavioral Health Urgent Care (BHUC) — A BHUC is a service containing Triage, Crisis Assessment, Interventions, Disposition and Discharge Planning. BHUC offers a safe alternative and diversion from the use of hospital emergency departments to address the needs of Members experiencing behavioral health crises.
Enhanced Crisis Response (Rapid Response Teams) — These services are directed to children and adults that are experiencing an acute behavioral health crisis that have presented in an Emergency Department and/or for step down from Inpatient. Services include crisis intervention, stabilization, and linkage to support and treatment.
Family Centered Treatment (FCT) — FCT is an evidence-based, intensive in-home treatment model specifically designed for children and adolescents who are at risk. This strategy treats the youth and their families with tailored therapeutic interventions. FCT is primarily aimed at maintaining their permanency goals.
High-Fidelity Wraparound — High-Fidelity Wraparound uses a customized and team-based approach to administer a coordinated and seamless set of services and supports for children and youth with complex emotional, behavioral, or mental health needs.
In-Home Therapy Services — A combination of evidence-based therapy services and coordination of care interventions provided in the home setting for individuals with complex clinical needs that traditional outpatient cannot adequately address in a time limited fashion.
Institute for Mental Disease — A hospital, nursing facility, or other institution of more than 16 beds that is primarily engaged in providing diagnosis, treatment, or care for adult members twenty-one and older diagnosed with mental health (MH) or substance use disorders (SUD) who require inpatient treatment.
Intercept Program — An in-home service that provides intensive diversion and stabilization work with families who have a youth in danger of out-of-home placement. This program provides short-term reunification services as well as for youth recently placed outside the home and returning directly home. A long-term reunification services for youth placed outside the home for an extended period. Assessments on all youth entering services and adoption stabilization for long-term placements are also provided.
Outpatient Plus (Intensive Outpatient for Mental Health) — This service is a combination of best practice outpatient therapy services, monitoring, support, and management of care interventions provided to individuals with complex clinical needs that traditional outpatient cannot adequately address.
Rapid Response Homes — Licensed therapeutic foster homes with a North Carolina Licensed Child Placing designation that provides emergency treatment, structure, stabilization, and supervision to children and youth who are experiencing a behavioral health crisis. This emergency service is intended to support family stability, prevent abuse and neglect, provide short-term treatment and prevent or minimize the need for out-of-home placements.
Residential services for individuals with complex needs for children with Intellectual and Developmental Disabilities (IDD) and co-occurring MH diagnoses — A short-term residential treatment service for members ages 5-21 focused on providing support for those with co-occurring conditions and a complex clinical presentation. The multi-disciplinary treatment team is comprised of staff who are trained to treat IDD, mental health, and severe behaviors.
Transitional Youth Services — Services support youth ages 16–21 with behavioral health and/or substance use disorders who are transitioning from child to adult systems of care. The service provides individualized, community-based interventions that promote skill development in areas such as education, employment readiness, housing, daily living, and behavioral health stability.

Well-child visits 1st to 15 months ($25): 6 visits between birth and 15-months
Well-child visits 15 to 30 months ($25): 2 visits between 15-30 months
Childhood Immunizations ($25): Complete all childhood immunizations by child’s 2nd birthday
Well-visit ($25): Complete 1 well-care visit per year; ages 3-21
Immunization for Adolescents ($25): Complete adolescent immunization before 13th birthday
Adult Preventive Visit ($25): Adults 22 and older who complete a preventive care visit; per year
Prenatal Visit ($25): 1 visit within 1st trimester and/or 42 days of enrollment into healthplan
Postpartum Visit ($25): 1 visit within 7-84 days after delivery
Oral Evaluation & Dental Services ($25): 1 dental visit for comprehensive or periodic; ages 21 and under
Tobacco Cessation Counseling ($25)
