- Prenatal and postpartum care visits
- Annual flu vaccine
- Diabetes A1c screening
- Childhood wellness visits
- Childhood immunizations (CIS)
- Adult well visit
- Immunization for adolescents (IMA)
- Tobacco cessation counseling
- Follow-up after behavioral health hospitalization
